AT Request for Assistance Form

Please fill out this form and email to one of us or send via Inter-School Envelope to DCMS (Attn: Assistive Technology).  If this is done a couple months before an IEP meeting, AT solutions can be explored and documented in the IEP.

RFB&D Student Certification Form

These two forms are to be used while ordering books in alternative format from Bookshare.org or RFB&D.  They are referenced in the Accessible Text Options for Students handout that was given out at the February 2009 Complex meetings.
